Visit the gofundme website "GoFundMe.com" and locate the campaign you wish to report

Scroll down to the bottom of the "Campaign" page

Select the "Campaign"

Click on "Report Fundraiser"

Select the "Reason" of report

Click the "Phone Number" field
Enter your phone number

Click the "Email Address" field
Enter the email address

Copy the "Fundraiser" url
Enter the fundraiser url

Click the "Fundraiser URL" field
Paste the fundraiser url

Click on the "Dropdown" and select the "Fundraiser" organizer

Click on the "Text" field
And please explain your relationship to the fundraiser organizer

Click on the "Dropdown" and select "Why would you like to refund" from given option

Click on the "Textbox" and tell us more about your concerns

Click on the "Choose File" button and upload any evidence you may have

Click the "Checkbox" and accept the terms and conditions
